Chronic and Complex Care
We look after people with chronic and complex conditions. If you live with a long-term condition (injury or illness), such as diabetes, asthma or heart disease, your GP here at Australia Fair in Southport can work with you on a Care Plan.
A personalised Care Plan sets out your goals, your treatment and who else is helping with your care. If required, your GP will include Allied Health referrals in your Care Plan. Your GP will talk with you about how often to review your plan.
Care Plan appointments take longer, so please book them by phone.
Fees and Medicare
GP Chronic Condition Management Plans and reviews are bulk billed for eligible Medicare card holders.
If your GP Chronic Condition Management Plan includes referrals to Allied Health Professionals, you may be entitled to a Medicare rebate for those Allied Health consultations.
